Healthy Utah Rebates

Healthy Utah is coming to Weber State University on August 7!

During your appointment, you will receive cholesterol and glucose testing, body composition analysis, blood pressure measurement, and a consultation with a health specialist, all free of charge for qualified employees and eligible spouses.  Everyone who participates in the testing session and answers an online health questionnaire will receive the “Know and Plan” $50 rebate!  After that, earn an additional $50 “Next Steps” rebate by completing your choice of wellness activities from a variety of PEHP Wellness activities and programs (see “Next Steps” rebate form).  Other rebates are also available for those with diabetes ($100) tobacco cessation ($50), and Wee Care ($50).  Visit our website for more information at www.pehp.org. 

***Please Note: You may only register for a Health Utah Testing Session once per plan year, OR 60 day prior to your last test date. Please log into your PEHP account, select "Wellness", then "Register for Testing" to view your testing session eligibility. ***
