Syllabus Statement

Disability Services recommends that the following statement be included in the syllabus for any course taught at Weber State University:

Any student requiring accommodations or services due to a disability must contact Disability Services in Room 181 of the Student Services Center or Room 256 at the Davis Campus. Disability Services can also arrange to provide course materials (including this syllabus) in alternative formats upon request.

Accommodations For Events & Activities

The department or organization sponsoring any event at WSU is responsible for providing any necessary accommodations. Individuals requiring an accommodation for an event or activity should contact the sponsor directly.

If you are a WSU Department, Club, or Organization, Disability Services recommends that the following statement be included in your event advertising (posters, flyers, etc.):

Weber State University, in compliance with the Americans with Disabilities Act, ensures equal access to all university functions, activities, and programs for individuals with disabilities. Please contact [person to contact] at [email/phone number] at least one week prior to this event to request accommodations in relation to a disability.
