Photo Release Forms for Events
Thank you for visiting this page to secure a photo release form for your activity, class or event.
Please note it is university policy that any and all photos/videos featuring students, faculty or community members have a signed release form before the images be used. If the event is a community event that is open to everyone to attend the university asks that you still be mindful and consider posting signs indicating photos are being captured. Photos of groups at these events that do not feature individuals do not need releases.

Weber State University Photo, Video and Liability Release Forms

Please refer to the Saftey of Minors campus training brochure, it is a supplement to PPM 5-37 and very similar to what can be found in Appendix B of the policy.
Hosting people age 18 and older
*College of Science faculty and staff are asked to submit photo/video release forms to the College of Science Public Relations and Marketing office by email at email@example.com. You may also contact this office with any questions regarding forms.
