A student with a complaint is asked to try to resolve them first with the person, office, or academic unit (e.g., a college) with whom the student has the grievance. We characterize such a complaint resolution process as occurring at the primary level or level 1 with most complaints being resolved at Level 1. 801-626-8107Process: When a student withdraws from classes, a Financial Petition may be submitted. Print, fill out, and sign the attached form. Attach supporting documentation. Supporting Documentation includes but not limited to the following:
- Medical reasons: Statement from the physician or hospital on official letterhead that provides:
- Date the medical condition was diagnosed.
- Date/Dates of treatment.
- Dates of hospitalization or confinement, recuperation period, release from treatment.
Please do not submit medical bills or insurance claims.
- Military deployment:
- Deployment papers if the student's military unit was activated and deployed during the semester.
- Other official legal documents that substantiate the inability to participate in courses during the semester due to the deployment or military commitments.
- Death in immediate family:
- Immediate family is defined as spouse, sibling, children, or parents.
- Will need to provide a certificate of death or obituary listing relationship to the deceased individual.
- Other: Special circumstances beyond the student's control may be considered.
- Documents to substantiate the claim and a letter, on official letterhead, from the source supporting the claim needs to be submitted.

Typical Issues Petitioned Through the Academic Petition Committee
Graduation: requests for a waiver or course substitution of the following university requirements: back-dating a Degree, Diversity- Residency (30 hours in residency at WSU for Bachelor's degree, 20 for an Associate's Degree), Scientific Inquiry, Total Hours, and Upper Division.General Education Requirements: requests pertaining to Breadth, Core, CIL, or all other general education requirements must go directly through the associated departments.
Major/Minor Requirements: request for exceptions to major, minor, or emphasis requirements must be submitted directly to the associated academic department.
Records: Retroactive add, cancelation, or withdrawal of a course(s), exceptions to academic records posting deadlines or processes, exceptions to academic renewal policy, and other academic records policies or procedure.
Registration: late cancelation, late withdrawal, modification of variable credits, changes in CR/NC or AU status, and exceptions to any registration policy or procedure.

801-626-7334Process: Students with a complaint would schedule a meeting with the Director of Student Involvement and Leadership (Tara), and based on the nature of the their concerns/complaint, the Director will then connect the student(s) with the appropriate advisor(s) related to the issue (i.e. student activities, WSUSA Senate, clubs and organizations, leadership programming, etc.). The Coordinators of SIL over each of these areas (the advisors) will then work with the Director of SIL and the student(s) to see if the issue(s) can be resolved.
