Life Insurance
Weber State University has contracted with Sun Life Financial to provide group life insurance.
The group policy number is 939764.
Through this policy, Sun Life provides:
Basic Employee Insurance
- Double indemnity coverage (two times your salary, two times your salary accidental)
- The university pays for the first $50,000 of your life insurance.
Basic Dependent Life Insurance:
- Pay .50 per pay period.
- Covers all eligible dependents.
- The employee is responsible for contacting Human Resources to cancel this benefit as well as any voluntary life insurance benefit on dependents who are no longer eligible due to aging out at 26 years old.
Voluntary Life Insurance:
- Increase your life insurance coverage through a voluntary plan.
- Options include coverage for yourself, your spouse, or your dependent children up to age 26. Some elected amounts do not require a medical questionnaire.
Accidental Death & Dismemberment (AD&D):
- Coverage of up to an additional two times your salary
