Name Change Process for Students

To make an official name change at Weber State, please complete the following steps.

In Person:

  1. Visit the Registrar's Solution Center windows, located on the first floor of the Student Services Building, Suite 101. At Davis campus, visit Davis Enrollment Services, located on the second floor of Davis Building 2, Suite 241.
  2. Bring a valid photo ID and a Social Security card reflecting the new name. Please note that a marriage license is not acceptable documentation.

By Email:

  1. Send an email request from your Wildcat email to registrar@weber.edu or registration@weber.edu
  2. Include your former name, new name, and W number.
  3. Include your reason for name change: marriage, divorce, or legal name change.
  4. Attach a scanned copy of a valid photo ID and a Social Security card reflecting the new name. Please note that a scanned copy of each original document is required; photos and screen captures are not acceptable documentation. Please also be sure that you have signed your Social Security card.

Please note that you will also need to change your Wildcat username to access the Testing Centers as well as ensure that your Wildcat email address matches your updated name. Please follow the process described below.

If you are a student employed at Weber State, please also follow the Name Change Process for Employees described below.

For students requesting a name change after already submitting their Graduation Application, please request a name change on your forthcoming diploma or certificate by emailing graduation@weber.edu from your Wildcat email.

Name Change Process to Add Preferred First Name

To add a preferred first name to your student record, please email your request to registrar@weber.edu from your Wildcat email. Your preferred first name will show in Canvas and Starfish.

To update your username and Wildcat student email to reflect your preferred name, please complete the following steps:

  1. Access the Wildcat Username Change Request Form
  2. Select First Name Change, Last Name Change, or Both Names Change.
  3. Complete the online form and click submit.

Username and email changes will be made by the IT Service Desk within 5 business days. This change will reflect in your Weber username and your email address, but currently does not change your Google display name for your email address. IT is currently working to change programming so that the Google display name reflects preferred name as well.

Students who would like their preferred first name on their Wildcard Student ID card can order a new card at the following locations:

  • Ogden Campus: Information Desk in the Shepherd Union Atrium (Union map).
  • Davis Campus: Wildcard Desk in Building D2, Suite 262 (Davis Campus map).

Students who are graduating can request that their preferred first name be used on their diploma or certificate and in the Commencement Program by emailing graduation@weber.edu from their Wildcat email.

Name Change Process for Wildcat Username & Email

To update your e-mail address to match your new name you will also have to change your Wildcat Username. This is optional and only needs to be completed if you would like your username and e-mail address updated.

To change your Wildcat username, please complete the following steps:

  1. Access the Wildcat Username Change Request Form
  2. Select First Name Change, Last Name Change, or Both Names Change.
  3. Complete the online form and click submit.

Please allow 5 business days for processing.

Name Change Process for Employees

To make an official name change with Human Resources, please complete the following steps.

  1. Visit the Payroll Office located in Miller Administration 110.
  2. Bring a valid photo ID and Social Security card reflecting the new name.
