Project Goal

Through a partnership with the Lions of Multi-District 28, Weber State University, and Primary Children’s Hospital Intermountain, our goal is to develop a year-round, community environment that inspires and supports healthy lifestyles among youth with diabetes in the Ogden/Clearfield and Salt Lake City Metropolitan Areas and their families through the Recreate, Educate, Advocate, and Climb Higher (REACH Weber) program. Using an Electronic Medical Record (EMR) system, our hope is to determine REACH Weber helps with more effective Time In Range (TIR) for youth.

Specific Project Objectives

Participants will:

  • Demonstrate competence of healthy recreation choices and diabetes knowledge.
  • Use autonomy to make healthy choices made regarding physical activity, nutrition, and lifestyle.
  • Initiate and maintain authentic friendships through a sense of relatedness.
  • Develop family citizenship by encouraging attributes important to being a member of a family.
