What Do I Report?
Report a Safety Concern
- If emergency medical care is needed, and it is between 7:30 a.m. and 5:30 p.m., report to the nearest IHC WorkMed. More information can be found on the Human Resources workers' compensation page.
- If there is an injury, illness or exposure, report to HR at 801-626-6032 or hr@weber.edu, and fill out an Individual Report of Incident (PDF Form or Online Form) and a Supervisor's Report of Incident (PDF Form or Online Form.)
- If a vehicle was involved or property was damaged, notify Risk Management at 801-626-6832 or risk@weber.edu, and fill out a Vehicle Accident Report, using either the PDF Form or Online Form.
- If there is a hazard, near miss or safety concern, such as spilled chemicals, complete the Near Miss and Safety Hazard Form.
- EHS may perform an assessment and/or corrective action report.
- If the incident is life-threatening or it is outside of the hours of 7:30 a.m. and 5:30 p.m., call 911 or go to the nearest emergency room.
