All appropriate documents must be completed before the date/time of your appointment. The name you type into these documents must match exactly the name you used when scheduling your appointment. If you have any questions about this process, call us at 801-626-6406.
Do not complete this paperwork if you do not have a scheduled Initial Consultation or Intake. If you would like to schedule an appointment, you may call our office.
For security reasons, we do not accept any Intake paperwork via email and we do not process any paperwork until clients arrive for their appointments. You are not considered a client of CPSC until your Intake appointment occurs.
We respect your privacy and the confidentiality of the information you share with us. If you would like us to release your confidential information to others, you must consent to this release in writing. All video therapy sessions will take place through a web-based software called Therapy Assistance Online (TAO). Once you have scheduled an Intake, an account in TAO will be created for you using your weber email. Check your email to finalize your account. *Please note that TAO has upgraded to a new version.
To begin your video therapy session, log in to TAO, and click on the camera icon in the upper right-hand corner at the time of your appointment.
